Staying In Your Own Home

Now more than ever, this option is available for seniors. Many services are available to help those who want to stay in their own home with a little assistance. Making decisions about services can be overwhelming, particularly after a stay in the hospital. Knowing just what you or your loved one needs, how much it costs and how to get the services can often be an overwhelming challenge. Many times these decisions need to be made quickly and it can be hard to find the information you need to make good decisions.

For people in both Stephenson and Jo Daviess Counties, the Stephenson County Senior Center is offering free, confidential Comprehensive Care Coordination - assessment and care planning for all senior adults, ages 60 and over, regardless of income level. The goal of this assessment and care plan is to help senior adults to continue to live in their own home and remain independent as long as possible.

You, your family, or caregiver can begin the process by simply requesting a visit by one of our Case Managers. The Case Manager will visit you at home to conduct this in-depth assessment, discussing what kinds of daily activities you can do on your own, as well as those that require help. For instance, after re­covering from a stroke, you may need home de­livered meals, transportation for medical appoint­ments qnd some help with household chores.

Once the Case Manager and you have talked about what sorts of things would help, we can help link you and your caregiver with the agencies that actually provide these sorts of services. There are many public and private services available for senior adults. Each has its own rules and eligibility requirements. Some have income guidelines and others do not.

In the end, it's up to you and your family or caregiver to decide what's best for you - what's going to be the most helpful for you so that you can continue to stay in your own home and remain as independent as you can.
